  11. What type of anesthesia do you use and why?
  12. Who will administer and monitor the anesthesia: a board certified M.D. anesthesiologist, a nurse or the operating surgeon?
  13. Where will the procedure be performed: office-based surgical facility, free standing ambulatory surgical facility, or hospital?
